duplicate title and description issue

While checking on google webmaster tool, I found the duplicate title and description issue in one of my web pages. The error sample show like this.

<!-- m --><a class="postlink" href="http://xyz.com/abc.php">http://xyz.com/abc.php</a><!-- m -->
<!-- m --><a class="postlink" href="http://xyz.com/abc.php/sitemap.xml">http://xyz.com/abc.php/sitemap.xml</a><!-- m -->

These 2 pages having same title and description. So it will show duplicate title and description issue in this. Anybody, please help how to fix this.

No. This is sample. If you want to know the exact url i will give you

If the pages aren't present then they will show 404 error message.

My suggestion is to you is that recreate your sitemap and then again submit the fresh one into google through the webmasters tool. It may take some time but it will be for good.

If those two pages have the same title and description, simply change title description of sitemap.xml.

and the second URL is wrong.
it should be like
<!-- w --><a class="postlink" href="http://www.domainname.com/sitemap.xml">www.domainname.com/sitemap.xml</a><!-- w -->
