Chitika and Infolinks are the top tier agencies here. Take a look at them if you haven't done that already.

2. Propeller ads
3. Infolinks... etc are the best ad networks for publisher...

As far as I know Chitika is closing down.
Infolinks is really great. But you will need very targeted traffic.
Becontext, Vibrant, Viglink all of these are good ones.
