Yes, services like GetResponse, Constant Contact provides templates as well, along with ESP services.

Aweber also provides them. But I think that for customizations they charge extra money.

Haven't used aweber. For us GetResponse works just fine.

Most of them provides, yes.

If they don't then all you have to do is ask.
