Right sidebar is the best place in my opinion.

Header too greedy, footer too lazy and left sidebar is just not great user interface.

You can put anywhere. As long as it doesn't interfere with the users experience.

You can also create a pop-up. In this way you don't have to put anything on your site's actual structure.

You can put them wherever you want. Just make sure its quite easily visible and also navigable.

Right sidebar at the top. Its the best position to keep the signup form in case of a small blog.
